HMG (Human Menopausal Gonadotropin), also known as menotropins, is a gonadotropin preparation containing both Follicle-Stimulating Hormone (FSH) and Luteinizing Hormone (LH) activity. Traditionally, HMG is purified from the urine of postmenopausal women. Modern highly purified HMG preparations commonly provide approximately a 1:1 ratio of FSH to LH activity.

HMG is particularly important in reproductive research because FSH and LH are two of the principal hormones controlling ovarian follicle development, ovulation, testosterone production and spermatogenesis.

How HMG Works

HMG essentially provides two complementary gonadotropin activities:

LH activity → Ovarian steroidogenesis/Leydig-cell stimulation

In females, these pathways contribute to follicular development and estrogen production.

In males, FSH supports Sertoli-cell function and spermatogenesis, while LH-like activity stimulates Leydig cells and supports intratesticular testosterone production.

HMG vs HCG

Although HMG and HCG are frequently discussed together, they have different functions.

HMG primarily provides FSH plus LH-like activity, giving it a direct role in follicular development and spermatogenesis.

HCG predominantly mimics LH activity through the LH/CG receptor. In male fertility protocols, HCG may therefore provide Leydig-cell stimulation while HMG contributes the additional FSH activity required to directly support spermatogenesis.
